Exploration of best practice scenarios through critical analysis of current research focused on evidence-based practice and practice-based evidence methods. Provides necessary theoretical foundations for the novice healthcare professional, upon graduation, to practice within the emerging roles of case manager, healthcare coordinator, chronic disease manager, healthcare navigator, utilization resource manager, and/or discharge planner specialist.
